Forms

Application forms for permits under the Agricultural Operation Practices Act (AOPA) have been separated into sections to enable the applicant to complete only those sections that relate to their individual application.  Step-by-step information and examples can be found in the corresponding guides on how to complete the AOPA application form.

Forms can be filled out online, printed and submitted to the NRCB, or downloaded and completed by hand for submission.  Please note that forms filled out online cannot be saved.

Operators who wish to request an amendment to an existing permit may use the application for amendment form below.

If you have any questions about the application forms, please contact an approval officer at an NRCB regional office closest to you.

Parties who wish to request a review of an NRCB approval officer's decision may do so by submitting a completed request for review form (found below) to the NRCB within 10 working days of the decision. 

Request for board review forms can be handwritten or typed; however, we require a signature on all request for board review forms.  If you choose to type in the form, please remember to print a hard copy and include your signature on the last page.  Should you require assistance, contact information is provided on the form along with contact details for submitting the form.
